There should be a section on the form that tells you that you are the registered keeper and gives you the option of admitting you were the driver or if you were not, asking for who was. Simply fill that in with the dealers details and send back.

I would ring the DVLA informing them you are no longer the keeper , giving the date and details of the dealer. They will update the system.
