get him to put in writing that he witnessed you couldn’t carry out the physical examination and what the assessor said.
let them know you’ll go all the way to a tribunal unless it’s corrected along with your score.
Again include all relevant medical evidence, stating why it’s impossible for you to do it.
Also phone and tell them you disagree and why.
your assessor has obviously made mistakes.
