I’m on a 3 year medical license, during Covid it took quite a while, I’d imagine it’s a lot quicker now, though they’ll probably write to your GP or specialist. As for insurance, I found it didn’t affect mine much (non Motability), in fact it came down a little. I think the insurance aren’t allowed to penalise you for it.
